NOTE: All volunteers must have proof of a TB test within the past 9 months. You must resubmit your TB form each semester that you volunteer for PEP.

1. Download the TB Test form for your doctor to fill out here. If your health-care provider is unable to fill out this exact form, please upload the form that they provided to you.

2. When the form is complete, upload a picture of it using the "TB Upload Form" on this page.

 

3. If you have any problems with the form, take a photo of it with your phone (or scan it), and forward the photo/scan here.
