PEP Certificate Templates

Note to Instructors:

1. Please fill out your course name, your facility name, and the name of the student on the template.

2. For in-person courses, please print out on
65-lb. beige card stock.

3. For virtual courses, please email the certificates to the facility supervisor.

PEP Evaluation Templates

Note to Instructors:

1. Please print copies of the Course Evaluation and distribute to your students on the last day of class.

2. After your students complete the evaluations, please collect the forms and then input the data here:
